Feedback Form

We are a learning organization. We value a risk-free learning environment and we believe that to teach is to learn. We value your feedback, as we believe that the training is the most powerful impact point in the Roots of Empathy program. The world is changing so fast, and so are the circumstances of education and childhood. Because of this, we will keep asking ourselves relevant questions in order to deliver the most relevant training.

We read every piece of feedback we receive, share the results organizationally, and make adjustments based on what we hear. We appreciate your authentic reflections and comments on our training, knowing that you have had a long day. Thank you.

Please take a few minutes to record your reflections and any useful information from your Mid-Year Training. The feedback you give us will help us deal with any outstanding issues in the region, bring forward personal information about instructors to the mentor, and make changes to the training.

Step 1: Please select name of Trainer completing the feedback and click "Submit".

Step 2: Answer Questions 1 & 2, and enter an ‘x’ or ‘n/a’ into each text box and click ‘Submit” to save a draft of your feedback form. This ensures that your answers will be ‘saved as you go’. You may now continue with completing your feedback making sure to click the submit button at the bottom of the page after each question to save your answers.

Step 3: To view, edit or revise answers already submitted previously, select your name from the dropdown menu and click submit to display the feedback previously submitted. Remember to click submit after any edits or revisions.
