Classroom Information Forms

Dear Instructors,

We are launching the new on-line Classroom Information Form (CIF) through the Instructor Dashboard.

Steps to complete your CIF (complete one form for each program you are delivering):

  1. Review your Profile Information found on the "My Profile" tab – Update if required
  2. Part One: School Information along with Curriculum Level – ‘ Save and Continue ’
  3. Part Two: Program and Classroom Details including Teacher Name – ‘ Save and Continue ’
  4. Part Three: Roots of Empathy Family Details – ‘ Save and Submit ’
  5. Inform your KPP once you have submitted a CIF for each program you are delivering.

NOTE:

  • An ‘ * ’ next to a field indicates that the field is mandatory and must be completed
  • If your Roots of Empathy family does not have an email address, please input the default email parent@rootsofempathy.org

Once the CIF is submitted you may edit it in the "EDIT CIF" tab. Click on the blue button in the first column next to the CIF ID number which will open a dialog box where you can make updates to the CIF. Only CIFs from the current Program/Programme year will be displayed in this table.

Submit your CIF(s) as soon as possible after your program begins. The CIF contains important information/statistics that will be shared with our funders and stakeholders to maintain accountability. Your timely submission of the CIF is very much appreciated.
